This position is hybrid remote.

JOB PURPOSE AND SUMMARY

This position works within the Healthy Communities Team to facilitate the implementation of 5-year Community Needs Assessment (CHA) and Community Health Improvement Plan (CHIP) cycles and other high-priority community health initiatives. This position oversees the coordination of complex community-level work plans – requiring well-developed project management and facilitation skills and the ability to work collaboratively with other CCPH program managers, external partners, and community leaders. This position will report to the Health Assessment and Evaluation Program Manager II.

Other responsibilities include understanding and promoting the public health mission of the department; providing courteous, respectful, and efficient customer service to all colleagues and partners; honoring diversity of all department employees and constituents; participating in department training and applying what is learned in our work; and striving for personal excellence in public health.

Work is accomplished by:

  • Using qualitative and quantitative data to facilitate the community-driven selection of public health priorities and goals
  • Collaborating with community partners to implement CHIP strategies and activities
  • Prioritizing primary prevention programming and community capacity-building
  • Supporting the development of local communities of practice

This work is to lead to:

  • Thriving community partnerships
  • Improved wealth-building and civic leadership opportunities for allied community partners
  • Progress towards identified CHIP goals (including measurable improvements in health and social outcomes)
  • Shared commitment to anti-racist policy development and implementation
  • Increased capacity building for partners representing diverse communities

The focus areas of this position are:

  • Facilitating the evaluation of CHIP strategies and activities with community partners and Healthy Communities team members.
  • Capacity Building with community partners (i.e., providing technical assistance and grant writing support for identified policy and program priorities).
  • Creating and maintaining relationships with diverse partners, including health-related national, statewide, and community-based organizations; community groups or organizations representing populations experiencing racial and health inequities; private businesses and health care organizations; Tribal Nations, and local, state, and federal government agencies and leaders.

This is a full-time non-represented exempt position.

This position has the option of a remote/hybrid work schedule possibility. However, the candidate selected must reside in OR or WA. No Exceptions.
