Identification Badges and Cards

An OHSU ID badge is required for entry to most OHSU buildings. New students typically pick up their ID badges at OHSU’s New Student Orientation, provided all pre-entrance badging requirements are complete. A government-issued photo ID is required. The first identification badge is free; students are charged for replacements.

If you need a new or replacement badge after New Student Orientation, follow these steps:

    • Your OHSU email address
    • Your name, exactly as you would like it to be printed on your badge.
      • If you wish to display a nickname, please provide your nickname first and your legal name in parenthesis. Ex. Jo (Joanne).
      • If you wish to only have the first initial of your last name displayed, please provide the initial first, and the last name in parenthesis. Ex. S. (Smith)
    • Indicate whether this is a new or replacement badge.
  3. A badge request will be submitted on your behalf. You will receive a copy of the request, and approval, at your OHSU email address.
  4. Visit one of OHSU’s Office of Transportation and Parking Customer Service Centers to get your badge once the request is approved. A government-issued photo ID is required.

Customer Service Centers are located in the Physical Plant Building on the Marquam Hill Campus, and in the Rood Family Pavilion on the South Waterfront Campus.  Please visit their website for hours of operation and directions.

PSU Student ID Card

A PSU ID card permits access to PSU’s Millar Library, the Academic and Student Recreation Center (ASRC), the Portland Streetcar, and campus buildings locked to non-students before, during, or after hours.  New and replacement badges are available from PSU ID Services. A government-issued photo ID is required. The first identification card is free; students are charged for replacements.
