Identification Badges and Cards

School of Public Health undergraduate students receive PSU ID cards. School of Public Health graduate students receive identification from both OHSU and PSU>.

OHSU Student ID Badge

An OHSU ID badge is required for entry to most OHSU buildings. New students typically pick up their ID badges at OHSU’s New Student Orientation, provided all pre-entrance badging requirements are complete. A government-issued photo ID is required. New and replacement badges are available from OHSU’s Office of Transportation and Parking. A student badge request form and government-issued photo ID are required. The first identification badge is free; students are charged for replacements.

PSU Student ID Card

A PSU ID card permits access to PSU’s Millar Library, the Academic and Student Recreation Center (ASRC), the Portland Streetcar, and campus buildings locked to non-students before, during, or after hours.  New and replacement badges are available from PSU ID Services. A government-issued photo ID is required. The first identification card is free; students are charged for replacements.
