We take student concerns, suggestions, and experiences seriously, and we are committed to uplifting and honoring student voices in meaningful ways. Whether you are sharing a concern, offering constructive feedback, or highlighting an area for improvement, your input helps us identify challenges, enhance resources, and resolve issues thoughtfully and effectively. What To Expect When Reporting
Complete and submit the form.
Provide as much detail as you are comfortable sharing to help us understand your concern.
Routing to the appropriate leadership.
Once submitted, your form will be reviewed and routed to the appropriate person — either the Associate Dean for Academic Affairs or the Assistant Dean for Student Affairs — depending on the nature of the concern.
Initial response timeline.
You can expect to hear back within 2–5 business days to acknowledge receipt and outline next steps, if contact information is provided.
Mandatory Reporting.
If your concern involves sexual misconduct, interpersonal violence, bias, discrimination, harassment, and retaliation of protected characteristics, SPH is required to report it to the PSU Office of Equity Audit and Compliance and/or the OHSU Office of Civil Rights Investigations and Compliance in accordance with institutional policies.
Anonymous submissions.
If you choose to submit your concern anonymously, we may not be able to follow up directly with you; however, your feedback will still be reviewed and considered carefully.

OHSU Office of Civil Rights Investigations and Compliance
The Office of Civil Rights Investigations and Compliance review and investigate reports related to policy violations, offers education, training and other resources, and confidential advocacy for employees related to bias, discrimination, harassment, retaliation, and interpersonal violence.
OHSU Confidential Advocacy Program
The Confidential Advocacy Program provides confidential support services to OHSU students who have experienced any form of sexual misconduct, including sexual harassment, sexual assault, intimate-partner violence, stalking, relationship/dating violence, and other forms — whether at OHSU or outside of the university.
PSU Equity & Compliance
PSU Office of Equity & Compliance is responsible for monitoring Portland State University’s Title IX compliance. They Reviews and investigates reports of bias, unlawful discrimination and harassment, failure to accommodate an individual’s disability, and violations of the Consensual Relationship Policy.
PSU Confidential Advocacy
What is the difference between an academic feedback and a non-academic feedback?
Academic complaints relate directly to the academic experience. This may include concerns about a course, course content, an instructor, grading, or other academic requirements or expectations.
Non-academic fomplaints involve issues outside of course instruction or grading. These may include concerns involving another student, general student wellbeing challenges, a specific incident that may intersect with academic or co-curricular experiences, or broader concerns about a program, services, or the institution.
How and where can I file feedback?
Students can submit feedback by completing the online Complaint and Feedback Form. This form is the primary way to share concerns, feedback, or report an issue with the OHSU-PSU School of Public Health.
Once the form is submitted, it will be reviewed and routed to the appropriate office or staff member to ensure your concern is addressed. If you are unsure whether your concern is academic or non-academic, you can still submit the form and it will be directed to the correct person.

Is this process confidential?
Yes. Information submitted through the Complaint and Feedback Form is handled with care and shared only with the appropriate staff members responsible for reviewing and addressing the concern. Responses are kept as confidential as possible while still allowing the School to review the issue and take appropriate action.
In some situations, certain types of concerns (such as those involving discrimination, harassment, or threats to safety) may be required to be reported to the appropriate PSU and/or OHSU office so they can be addressed according to university policies.
Who should I contact if I have a financial aid question?
Financial aid questions are handled by the financial aid office at the university where you are matriculated.
- Graduate students should contact the OHSU Financial Aid Office for assistance.
- Undergraduate students should contact the PSU Financial Aid and Scholarships Office.
If you are unsure which office to contact, please review your student status or reach out to the appropriate financial aid office for guidance.
What should I do if I have questions or concerns about academic misconduct?
If you have questions about academic misconduct or need to report a concern, please submit the Student Complaint and Feedback Form. This form allows you to share information about potential academic integrity issues so they can be reviewed and addressed by the appropriate office.
Once submitted, your concern will be routed to the appropriate staff member for review and follow-up.
